Any employer would have a blank one, so JVB could have gotten one (even if not from Reilly) and typed Reily's name and tax ID number on it.
Form C is for the employees personal record. Form A is for the employer, maybe?
The top form in the image doesn't go with the line of holes to the far right. There is too much paper between the edge and the hole of the far right line of holes for the top part to come from the rest of that book. This photograph is a unused copy B on top of something else that looks (judging by the bleed thru) more like a real W2.
