But Mr Everingham confirmed to the Mail today that the man she refers to in the book as 'the Patron' is in fact Lord Hamlyn, who died in 2001 after suffering from cancer, pneumonia and Parkinson's disease.
'I promised her I would leave his name out, which I did, but there have been whispers all around Sydney for years that he was her secret lover,' said Mr Everingham. 'It was inevitable that after the book came out those whispers would grow louder.
'Her life with him spanned many years after he was introduced to her in 1977 through a businessman he met on one of his annual trips to Australia.

'When he met her he was in his early 50s and married, of course. He had so much money, though, that he could afford to fly her anywhere he wanted and enjoy all manner of sexual liaisons with her.'
Speaking at his Melbourne home, Mr Everingham added: 'She wanted me to keep his name out of it because she still receives money from his estate and doesn't want that to come to an end. Of course it was going to be rather embarrassing for his family, too, should his name leak out.
'He was a very discreet man and worked very hard at keeping their meetings private. To ensure that, he paid her a very large amount of money for her discretion and to keep her happy.
'She was basically at his beck and call at all times - he didn't want her working and not to be available.
'Although he managed to come to Australia at times, more often than not it was she who travelled to see him; Singapore, Europe, you name it and she would fly to be with him and engage in their sexual stuff.
'He even put her up in an apartment in London while he was in another and then they would team up.
'When they weren't physically together, he loved to call her up on his car phone and she would answer her own car phone, which he made sure she had.
'Whenever she received the call to join him, she would pack a bag with unimaginable sex toys.'
Mention the name Madam Lash around Sydney today and long-term residents in the harbour city picture a woman in dominatrix gear, complete with whip - a lingering pose that she was always proud to adopt for photographers.
But she also dressed to kill for other big names, such as Clyde Packer, the elder brother of the late media baron, Kerry.
Today, at the age of 64, she still throws the wild parties for which she was famous, despite having a mastectomy and two hip replacements.
But since the launch of her biography she has gone to ground, furious, say friends, that Lord Hamlyn's name is now doing the rounds - just as she did when he was alive
