This is wrong on both counts. There have been cases of fixing in the world cup and champions league in recent times - both bigger stages than the EPL.
Anyway there are so many layers and levels of fixing it is hard to put it all under the word match fixing. A bit like when people put all drugs under the same umbrella, you can hardly class a joint the same as heroin.
The same with match fixing, you can have a manager playing a much weakened team from his squad on purpose to "surrender" the game. Or you can just simply have everyone involved in a big mafia operation to produce a pre determined result like this.
